I just used L3DT and it's pretty dang awesome, not sure if it has a pipeline to blitz3d though...

Depends on the kind of terrains you need to make..
L3DT is quite simple to control and the results are awesome..
T.Ed is a very good terrain editor, very uncomfortable interface, but the techniques both of them uses are quite different.. The first one uses a "mega-texture" while the second one uses texture splatting.
In terms of speed and simplicity, "Panorama" by Jeppe Nielsen is amazing, with much diference is the fastest to deal with and the results are quite controlables, but it's a shame it doesn't offers a great number of options or features, even with that, is quite recommendable.
You can check also the terrain generator of Gile[s] 2.0... It seems very promissing, but I've not played anough with it.
I've worked a lot to obtain good results and I can strongly recommend L3DT and Panorama, T.ed is also an option but very uncomfortable to me. All depends on your patience. ;)

Me is writing a own terrain engine. Heres a link for a 5000x5000 area.

it renders as you walk around.. in realtime.

you may use source as you wish. 2 exe is included.

to generate height map i use GIMP.

I've heard of L3dt from the Garage Games website.
The only two things that keeps me from buying it is

1: I like the terrain editor that comes with Torque, so I think spending more money on a editor just to get bigger terrains is a waste of money.
2: I am uncertain of its support for Blitz3d or 3D Gamestudio. If someone knows that L3DT can support these two packages (and prove it, either by documention links or simple demos) as well as Torque, then I'll reconsider my decision.

last i checked l3dt doesn't export b3ds but you can use the heightmaps and colormaps it produces and get good results.
